By doing so, the caregiver(s) will be honored with a special recognition and will receive a copy of your thank you note. Your gratitude will easily be turned into lasting support that will impact future patients and families for years to come.

To send your gift in honor of a caregiver by mail, please include the following information:
- Your name and address
- The caregiver’s name that you wish to honor
- The IU Health Goshen program to which your gift should be designated
- A note to the caregiver to be honored
