Generous hearts inspire thoughtful gifts
Indiana University Health Goshen’s tribute program provides the opportunity to recognize someone special to you. Tribute gifts may be made to honor a loved one’s recovery, honor a physician, nurse or other caregiver. A gift may also be designated for any occasion – to celebrate a birth, anniversary, holiday – or used to express appreciation or love. Many people find great comfort in remembering a friend or loved one who is deceased with a special gift to IU Health Goshen.
The family or individual you designate will be notified of your thoughtfulness through a personalized, acknowledgment, although the amount of the gift is not included.
To send your tribute gift by mail, please include the following information:
- Your name and address
- The person’s name to whom you are paying tribute
- The name and address of the person who should receive an acknowledgement card
- The IU Health Goshen program to which your gift should be designated
- Please specify whether your gift is in honor or in memory of the person to whom you are paying tribute
